L'équipe IMIS a été constitué à la création de ICube par des membres de l’ancienne équipe « Imagerie in vivo » du Laboratoire d’imagerie et de neurosciences cognitives (LINC) renforcée par l’arrivée de scientifiques issus du domaine de l’étude des pathologies cérébrales, de l’instrumentation en Résonance magnétique nucléaire (RMN) et Imagerie de résonance magnétique (IRM). C'est une équipe ayant des compétences complémentaires en imagerie et en santé. Elle associe, sur le site campus médecine de Strasbourg (bâtiment ''[http://plateforme.icube.unistra.fr/imagines/index.php/Acces]'' au sein des Hôpitaux civils de Strasbourg et du CHU de Hautepierre), des chercheurs, des ingénieurs et des médecins, ayant la volonté de travailler en équipe dans le but de développer des thèmes et des projets de recherche pluridisciplinaires originaux.<br /><br />

Le projet de l’équipe est d’identifier et valider de nouveaux marqueurs pertinents en imagerie IRM et en métabolomique, si possible quantitatifs, en utilisant ou développant de nouvelles méthodologies pour l’IRM et la métabolomique et ceci pour mieux comprendre, prédire, diagnostiquer et guérir les troubles neurologiques liés à l’âge (Alzheimer, Corps de Lewy), les troubles psychiatriques (dépression et catatonique, les troubles neuro-cognitifs de la prise alimentaire (obésité infantile) et des pathologies en oncologie. <br /><br />

Il se décline en 3 thèmes et concerne : le phénotypage métabolique et la caractérisation de tissus cérébraux tumoraux utilisables en neuro-chirurgie (thème 1) ; la caractérisation anatomo-fonctionnelle de la connectivité cérébrale utilisable en neurologie du développement et du vieillissement, et en psychiatrie (thème 2) ; le développement de l’imagerie multi-modale quantitative pour des applications pre-cliniques sur des modèles murins des pathologies humaines (thème 3).<br /><br />

Cette configuration de l’équipe offre la possibilité de développer des travaux spécifiques sur le petit animal (métabolisme et connectome du cerveau de la souris par exemple) et renforce la possibilité de conduire des recherches translationnelles Homme/petit animal pour répondre à des questions biomédicales portées par les chercheurs et les hospitalo-universitaires de l’équipe dans des disciplines telles que la neurologie/neurosciences, la radiologie, et la biophysique. Les développements méthodologiques, la création des cohortes, s’appuient sur les équipements de la plate-forme d’imagerie innovante en santé « [http://plateforme.icube.unistra.fr/imagines/index.php/Accueil Imagines] » (IRM Homme et petit animal, imagerie optique, labellisée IBIsA et la plateforme de Métabolomique des Hopitaux Universitaires de Strasbourg. Ils se font également en collaboration avec les autres équipes de ICube.<br />

Les travaux de recherche en IRM sont principalement orientés vers le développement de méthodologie IRM permettant accéder à des informations quantitatives (on parle d’IRMq), principalement celles associés à la structure et composition des tissus, pour des applications en recherche pré-clinique et clinique. L’originalité de notre recherche en IRMq réside dans l’utilisation des techniques d’imagerie rapide t multi-paramétrique, à haut-champs (3T) et à très haut-camps (7T et plus) associées à la modélisation du signal IRM. <br /><br />

Actuellement, sept protocoles de recherche utilisant l’IRMq sont en cours à Strasbourg. Sept autres protocoles IRMq, dont six multicentriques, sont en cours de préparation. La participation à des réseaux nationaux (réseau d’experts CATI et France Life Imaging)et international (COST Action BM1304) permet la divulgation, validation et harmonisation de la méthodologie en IRMq avec d’autres centres et son application dans les protocoles multi-centriques nationales et européens. <br /><br />

En collaboration avec l’Institut de Myologie (Laboratoire RMN), à Paris, nous travaillons sur le développement des nouvelles techniques d’IRMq, pour les études des troubles neuromusculaires. Récemment nous avons proposé une nouvelle approche qui permet une réduction du temps d'examen d'environ 50% par rapport aux méthodes classiques, tout en reposant entièrement sur une séquence d'IRM standard, disponible sur la majorité des IRM cliniques. Ce travail, publié en page de couverture dans le journal NMR in Biomedicine, a reçu deux prix lors de la conférence ISMRM en 2015.<br /><br />

Après l’arrivée de l’IRM 7T « petit animal » (mis en activité en 2014) nous avons démarré un nouveau projet de recherche en IRMq, en collaboration avec la société Bruker BioSpin MRI, Ettlingen, Germany : « La quantification directe de la myéline avec les séquences à temps d’écho ultra-court » (sujet de thèse de L. Soustelle, en cours). La transposition de cette méthodologie sur l’IRM clinique (3T) est un de nos objectifs pour le prochain quinquennat.<br /><br />

L’utilisation de l’imagerie optique en routine médicale et en chirurgie ouvre de nouvelles perspectives cliniques : chirurgie mini-invasive, télémédecine... Elle offre également de nouveaux facteurs de contraste et peut compléter utilement les méthodes conventionnelles d’imagerie anatomique ou fonctionnelle. Pour être utilisées en clinique, ces méthodes optiques doivent résoudre, ou contourner, les problèmes liés à la très forte diffusion et à la faible pénétration, limitée par cette diffusion et par l’absorption, de la lumière dans les tissus. L’originalité de notre équipe est d’avoir exploité le temps de vol des photons dans les tissus pour mesurer plus d’informations afin d’améliorer la reconstruction des images ou d’extraire les informations pertinentes en évitant cette étape de reconstruction, longue et difficile. <br /><br />

C’est ainsi que nous avons développé plusieurs instruments de spectroscopie et de tomographie proche infrarouge avec des techniques d’illumination pulsée et de détection résolue en temps de vol. Durant ce mandat, l’accent a été mis plus spécialement sur leurs applications précliniques et cliniques, tant en neurologie qu’en cancérologie. <br /><br />

L’analyse des données issues du tomographe optique préclinique a bénéficié de plusieurs avancées portant sur l’exploitation des profils temporels, et non de leurs moments, permettant d’améliorer la qualité des images obtenues tant en lumière diffusée qu’en fluorescence.<br /><br />

La caméra Spirit (Spectroscopie Proche Infra-Rouge par Imagerie Temporelle) a été couplée à un laser continuum équipé d’un filtre optique passe-bande. Le gain en puissance ainsi obtenu permet d’envisager la détection sans contact d’activations cérébrales préfrontales et l’imagerie per-opératoire par fluorescence 3D. Les expériences sur objets tests et les simulations effectuées ont démontré le potentiel de ce dispositif, tant en neurosciences qu’en cancérologie.<br /><br />

Des dispositifs à illumination continue ou modulée à basse fréquence ont également été développés. Plus simples, robustes et moins onéreux, ils facilitent le transfert vers la clinique ou la commercialisation. Ces équipements ont été exploités pour la recherche de cancers et de ganglions sentinelles, par détection d’agents de contraste commercialisé avec des photons d’excitation ou de fluorescence ou par détection de nanoparticules magnéto-optiques.<br /><br />

Avec l’acquisition de cohortes sur la plateforme Imagines (AlphaLewy, ..), le chercheur fait face à des images de plus en plus volumineuses avec une nouvelle dimension - le temps - et la multi-modalité (multi-séquences). Une analyse visuelle qualitative ne suffit plus pour exploiter au mieux ces données. Nous avons axé nos recherches plus particulièrement sur la détection statistique de changements sur des séquences temporelles d’images (volumétrie, imagerie de diffusion-DTI) et l’analyse des signaux obtenus en IRMf de repos.<br /><br />
<br />
Un premier travail a permis une compréhension fine des composants nécessaires à l’estimation de l’atrophie cérébrale. Les images de diffusion (IRMd), permettent d’apporter des informations nouvelles sur les microstructures locales des tissus, par rapport aux modalités d’IRM anatomiques plus classiques. Par contre les IRMd sont des images multivariées (vectorielles, tensorielles). Pour la détection automatique de changements sur des séquences temporelles d’images IRM de diffusion, intra-patients, notre recherche a donc porté sur le développement de nouveaux modèles et tests statistiques. L’étude d’une base de données constituée d’examens longitudinaux de patients atteints de neuromyélite optique a montré qu’il se produisait des changements au sein de la substance blanche d’apparence normale (aucune modification en imagerie morphologique) qui pouvaient être corrélés avec l’évolution des capacités cognitives ou motrices des patients. Ces méthodes de détection de changements en IRMd pourraient ainsi offrir de nouveaux biomarqueurs pour le suivi des pathologies neuro-dégénératives.<br /><br />
<br />
En imagerie par résonance magnétique fonctionnelle (IRMf) de repos, nous avons travaillé sur l’analyse et l’interprétation au niveau du sujet, qui s’avère nécessaire dans un cadre diagnostique où l’individu prime. L’Analyse en Composantes Indépendantes (ACI) permet de décomposer les volumes cérébraux, acquis lors de l’expérience IRMf, en cartes d’activation et décours temporels associés. Une chaîne de traitement automatisé a été développé pour la pré-sélection des réseaux de repos chez un sujet, avec pour contrainte une sensibilité de 100 % dans la détection des réseaux cérébraux spontanés [4-STRA15]. Enfin nous sommes en train de développer des méthodes basées sur des modèles de Markov cachés (MMC) et plus largement celui des réseaux bayésiens dynamiques pour l’analyse de la dynamique de la connectivité fonctionnelle (DCF), c’est-à-dire l’étude des modes d’interaction de ces réseaux dans l’espace et dans le temps. L’application retenue vise à détecter et à caractériser le cas échéant le changement de la DCF avant et après prise de médicament, ainsi qu’avant et après prise d’un placebo chez un sujet unique. Un autre travail a concerné l’indexation de spectres de Résonance Magnétique Nucléaire (RMN) de type HSQC et d’Images de Résonance Magnétique fonctionnelle (IRMf). <br /><br />

<div>Le métabolome, produit final de l’expression des gènes, est défini comme l’ensemble de petites molécules qui, avec les protéines, gèrent le fonctionnement de la cellule. Son utilisation dans le domaine biomédical a comme objectif de surveiller l’état métabolique cellulaire à un moment donné, de mieux comprendre les interactions biologiques complexes dans les tissus, d’étudier l’influence de l’environnement, de l’alimentation ou des médicaments et d’enfin identifier des biomarqueurs liés aux conditions spécifiques de pathologies. En comparaison avec les autres systèmes en « omique » (génomique, transcriptomique, protéomique), la métabolomique est un domaine relativement récent et en plein essor depuis le développement de la technique HRMAS (high resolution magic angle spectroscopy) en RMN (résonance magnétique nucléaire). Cette technique, même si sa sensibilité est largement inférieure à la spectroscopie de masse, permet de travailler à haut débit sur des échantillons tissulaires intacts et d’avoir une vue globale sur le système biologique choisi. Les méthodes d’analyses statistiques multi-variées (PCA, PLS) permettent ensuite d’étudier les changements métaboliques dans un domaine de pathologie par rapport au tissu sain et donnent la possibilité d’identifier des empreintes spécifiques de diagnostic, de pronostic et/ou de la réponse thérapeutique. <br /><br />

Labellisé le 20 décembre 2006 par le Pôle de Compétitivité Innovations Thérapeutiques de la Région Alsace (nouvellement Alsace BioValley) à travers un consortium entre les Hôpitaux Universitaires de Strasbourg, l’Université de Strasbourg, le CNRS et Bruker BioSpin, le projet CARMeN a été financé à la hauteur de 2,7 millions € pour créer une base de données de la métabolomique en cancérologie, avec une stratégie qui consiste à utiliser l’empreinte métabolique de la tumeur, pour l’identification précoce des cancers à haut risque.<br /><br />
Grâce à ce financement, l’implantation du premier spectromètre RMN HRMAS (500 MHz) dans le milieu hospitalier a pu être réalisé fin 2007 au sein même du Service d’Anatomie Pathologique à l’Hôpital de Hautepierre. La proximité avec le Service d’Anatomie Pathologique, où toute la logistique de la tumorothèque fonctionne, nous a permis d’accéder rapidement et en toute sécurité à un grand nombre d’échantillons provenant des salles d’opération, de mettre en place des circuits garantissant des délais d’ischémie très faibles et une qualité exceptionnelle d’échantillons tissulaires après vérification histopathologique. Dans la période 2011-2015, des collaborations avec les équipes médicales et scientifiques ont été développées afin d’exploiter les données de la métabolomique en cancérologie avec plus de 5000 échantillons analysés par notre équipement à partir des échantillons provenant des salles d’opération des Hôpitaux Universitaires de Strasbourg et d’autres CHU (Marseille, Nancy, Lyon, Besançon), mais également de l’expérimentation animale. <br /> <br />

Fort de l’expérience CARMeN, nous avons développé au cours de ce quinquennat un nouveau projet dont l’objectif est de proposer une analyse du profil métabolique par RMN HRMAS des tumeurs cérébrales pendant l’intervention chirurgicale dans un lapse de temps suffisamment court (de l’ordre de 15 min) susceptibles de compléter l’analyse histopathologique en extemporanée. Le projet a été labellisé le 11 décembre 2012 par le Pôle de Compétitivité « Alsace BioValley » et est financé dans le cadre d’un Projet BpiFrance ISI (27 mars 2013) à la hauteur de 12,7 millions €, à travers un consortium entre les industriels Bruker BioSpin et Ariana, les Hôpitaux Universitaires de Strasbourg, l’Université de Strasbourg et le CNRS.<br /><br />
Actuellement, nous sommes capables de fournir des informations sur le degré de malignité de la tumeur et de détecter la présence de mutation du gène IDH indicatrice de meilleur pronostic. Nous analysons également plusieurs prélèvements provenant de la cavité d’exérèse afin de détecter d’éventuelles infiltrations tumorales résiduelles. Nous envisageons de projeter le résultat de chaque analyse sur le système de neuronavigation, avec un code couleur, directement sur l’IRM correspondant à la localisation du prélèvement. Cette approche que nous avons qualifier de « chirurgie guidée par la métabolomique » est donc susceptible de modifier l’intervention chirurgicale en cours et d’orienter le neurochirurgien vers une extension de l’exérèse si nécessaire. <div>L'objectif de ce thème à moyen ou à long terme est la recherche de biomarqueurs diagnostiques, pronostiques ou thérapeutiques de maladies neurologiques ou psychiatriques pour lesquelles une lésion cérébrale est avérée ou supposée sans pouvoir être démontrée avec les techniques d'imagerie actuelles. On parle d'anomalies cryptiques.<br />
S'il est possible que l'imagerie n'atteigne jamais la résolution ou la sensibilité souhaitée pour mettre directement en évidence ces anomalies, en revanche, les conséquences de celles-ci sur l'organisation anatomique et fonctionnelle du système nerveux central sont à notre portée. Il est possible d'évaluer ces dernières avec les techniques d'imagerie actuelles et de les corriger de façon ciblée et non invasive (médicaments, stimulation par rTMS, HI-FU). L'application de ces biomarqueurs à titre diagnostic, pronostic ou thérapeutique se ferait à l'échelle du sujet unique ouvrant ainsi la voie à une médecine personnalisée. <br><br />

== Une stratégie alternative pour combler ces lacunes ==<br />
Nous souhaitons nous intéresser à l'ensemble du cortex en une seule acquisition afin de rendre cette approche applicable à un grand nombre de problématiques avec un minimum d'adaptation. De plus, nous voudrions disposer de mesures dont le niveau de quantification soit absolu et dont le résultat puisse être suffisamment fiable pour autoriser une conclusion à l'échelle du sujet unique.<br />
=== Volet fonctionnel ===<br />
Nous proposons une mesure fonctionnelle de l'activité cérébrale qui soit suffisamment diversifiée. Deux conditions émergent en pratique : condition de repos (yeux fermés sans bouger) et/ou d'implication de fonctions cognitives multiples (jeu 3D intégrant traitement visuel dont lecture, traitement sonore, gestion émotionnelle et fonctions exécutives).<br><br />
Nous utiliserons une séquence que nous avons mise au point, sensible à la fois au contraste ASL (Arterial Spin Labeling) et au contraste BOLD (Blood Oxygen Level Dependent). La première apporte la quantification absolue, la seconde la sensibilité. Mais au-delà, nous utiliserons des modèles physiques et physiologiques pour tirer des informations comme la concentration en hémoglobine désoxygénée. Il s'agit là d'un premier niveau de ce que nous entendons par « intégration », c'est-à-dire d'une utilisation de la multimodalité qui ne se contente pas d'une simple addition des résultats, mais qui par leur combinaison permet de tirer des informations supplémentaires, non disponibles si mesurées séparément.<br><br />
L'activité cérébrale sera évaluée à l'échelle des réseaux et des régions qui les composent en utilisant des analyses multi-variées dont certaines sont en cours de mise au point. La plus prometteuse semble être l'analyse en composantes indépendantes dans le domaine spatial qui permet un découpage fonctionnel des régions corticales. Cette analyse ne nécessite aucun modèle préalable et permet d'obtenir des résultats beaucoup plus reproductibles que l’analyse classique par SPM (kappa au voxel près de 0,6 et 0,08 respectivement). De plus, elle permet d'identifier des réseaux pathologiques(épilepsie, hallucinations). Il est possible aussi que l'absence de certains réseaux ait du sens(coma, Alzheimer, autisme, …) <br><br />
Des optimisations sont envisagées comme la compensation des mouvements en temps réel au cours de l'acquisition par l'utilisation de capteurs magnétiques externes. Ceci permettra d'étendre plus facilement ces approches aux cas du jeune enfant et du nourrisson.<br />

Sur la base d'IRM 3D haute résolution maximisant le contraste substance grise/substance blanche (inversion récupération), nous cherchons à mettre en évidence des anomalies de gyrification ou d'épaisseur de cortex à l'échelle du patient unique vs. un groupe contrôle.<br><br />
D'autre part, nous utiliserons des données en imagerie de diffusion à haute résolution angulaire (HARDI) pour déterminer la matrice de connectivité de tous les voxels de la substance grise. Sur cette base nous recherchons des techniques de regroupement de ces voxels pour définir des aires de connectivité anatomique conjointe. Cette approche se base sur la définition neurophysiologique d'une aire corticale comme étant l'ensemble des colonnes corticales ayant une connectivité commune. L'évaluation de la connectivité anatomique en HARDI sera confrontée à la réalité terrain par une étude anatomo-histologique et en imagerie de polarisation ainsi que par des approche alternatives en IRM (SWI ou imagerie pondérée en susceptibilité magnétique).<br />

== Constitution d'un atlas ==<br />
Pour parvenir à déterminer des biomarqueurs robustes et parfaire l'intégration entre les deux modalités, il nous faut disposer d'une base de données suffisamment large. Nous allons développer un atlas descriptif des réseaux fonctionnels et des aires qui les sous-tendent au travers des âges avec l'ambition d'y repérer des règles et des invariants dans leur structure, leur développement et/ou leur involution. Cela débute par la mutualisation d'une large base de données de sujets sains s'étendant du nouveau né au sujet âgé.<br><br />

Ce premier atlas sera doublé sur la base des mêmes sujets par un atlas des aires définies par leur connectivité anatomique. Nous décrirons là encore leur développement et leur plasticité liée à l'âge et confronterons cet atlas anatomique à l'atlas fonctionnel.<br />
=== Intégration anatomo-fonctionnelle. ===<br />
L'acquisition conjointe de l'anatomie et de la fonction autorisera un second niveau d'intégration : entre l'anatomique et le fonctionnel. Un atlas anatomo-fonctionnel, permettant les comparaisons inter-sujets et encore plus des comparaisons d'un individu avec un groupe contrôle, en sera le référentiel commun. Jusqu'à présent la procédure consiste à normaliser les cerveaux des participants, c'est-à-dire à les déformer pour qu'ils correspondent à un cerveau dit « normal » pour lequel une parcellisation en aires a été définie sur des bases cyto-architectoniques. Cette procédure de déformation n'est précise qu'à 1 ou 2 gyri près, mais l'optimiser ne serait que d'une aide limitée car la gyrification est variable d'un sujet à l'autre et surtout les aires sont disposées de façon variable par rapport à l'anatomie gyrale. Cet atlas anatomo-fonctionnel sera le premier de ce type en neurosciences humaines. Outre une description statistique de l'anatomie fonctionnelle chez l'homme adulte, il permettra de répondre à de nombreuses questions : quel est le degré de superposition de ce découpage entre les individus, quelle relation avec la gyrification, quelle plasticité liée à la fonction …<br><br />
Des acquisitions semblables, adaptées au nourrisson et à l'enfant permettront de décrire la maturation fonctionnelle du cortex et de la substance blanche : le découpage est-il préexistant, quelle est sa flexibilité, la myélinisation précède-t-elle toujours la fonction, effets de la prématurité …<br />
A l'autre bout de l'échelle des âges, d'éventuelles modifications pourraient être mises en évidence dans le vieillissement normal : observe-t-on une réduction de la complexité ou des premiers signes de vicariance …<br><br />
Cette base de connaissance permettra d'étudier les pathologies qui s'accompagnent d'une réorganisation de la connectivité cérébrale ainsi que les modes de vicariance du système : pathologies neuro-développementales en tête (comme les autismes ou les schizophrénies), mais aussi dans les pathologies neuro-dégénératives.
